How do I apply for an EXTERMINATION PERMIT?
The owner or his representative must come to the Health Department at 605 Washington Street to complete a short application and submit payment for the permit. In the future you will be able to click HERE to download and print the form if you would like to fill it out ahead of time. Applications submitted without payment will not be processed.
